Great Wall Chinese, 2905 N 7th St, tracked, July 15, failed. The facility will be reminded of employee health forms and proper procedures. The food establishment operator failed to display an original and valid PDA food employee certification in a location conspicuous to the consumer. Garbage is not stored in receptacles or waste treatment units inaccessible to insects and rodents. Soup cans used as scoops stored in dry goods. The prep table with flour, a food contact surface, had food residue and was not clean to sight and touch. Non-food contact surfaces are not cleaned at a frequency to prevent the accumulation of dirt and soil. No certificate displayed. Old, unused material stored in the area should be removed from the food facility. The food establishment area is extremely dirty, dusty and needs cleaning. The operator of the food establishment must be the responsible person or must designate a responsible person and must ensure that a responsible person is present at the food establishment during all hours of operation. Using cardboard on the counter is not an approved surface. Screen door is broken allowing entry of insects and rodents and needs to be repaired/replaced
